Carers Week celebrates and recognises the vital contribution of carers. There are an estimated 745,000 identified adult carers and 44,000 young carers under the age of 18 in Scotland.
Carers care for relatives, friends and neighbours with longstanding illness or disability. Many of us will have our lives touched by caring and three in five of us will be carers in our life time.
Building communities which support carers to look after their loved ones well, while recognising they also have individual needs is the focus of the Carers Week 2017.

‘Let’s talk about planning for outcomes’ workshop
We are supporting the implementation of the Carers (Scotland) Act 2016 by supporting workforce development and as part of this we held a workshop with NHS Education for Scotland which focused on good conversations and planning for outcomes.
On 5 June over 50 health and social care practitioners from across Scotland came together to share learning from outcomes focused support planning in different practice settings. They were sharing experiences and learning from having good conversations about outcomes and what contributes to this.
In celebration of the following day’s ‘What Matters to you?’ day we started with a focus on that very question: to get to know each other and start the conversation about good conversations and planning for outcomes.Colleagues also heard more about the Carers (Scotland) Act 2016, which comes into effect on 1 April 2018, from the Scottish Government Carers Policy Adviser, Gordon Dodds. He outlined how this will work alongside other initiatives and programmes to support carers and the people they care for, including transformation of primary care and reform of adult social care and the social security system. The driver for change is providing choice and control through self-directed support.
The workshop was an opportunity to explore common themes across carer support planning, self-directed support, anticipatory care planning and discharge planning.
Michael Tornow, Senior Health Improvement Officer – Public Service Reform, NHS Health delivered an informative session on dementia and equality followed by learning from work on personal outcomes in dementia, post diagnostic support which was shared by Stephen Lithgow, Associate Improvement Advisor, Focus on Dementia, Improvement Hub.

How you can get involved in the conversation
Good conversations and planning for personal outcomes is just one of the themes discussed at the quarterly Personal Outcomes Network (PON) meetings. The PON is a network open to all which aims to offer a safe place for reflection and discussion of practice issues raised by members themselves. Members share a common passion for developing and implementing personal outcomes approaches locally and nationally and come from all sectors across health, social care and housing. Practice is supported through sharing stories, learning, resources and evidence. The PON has produced a Personal Outcomes Values and Principles statement which members have used to help inform and embed personal outcomes approaches in their organisations.
