When you make a fitness to practise referral about a registered worker there’s key information we need to know and it’s important that you give us as much of it as possible right from the start.
The more information you can provide, the quicker and easier it is for us to decide whether a matter meets our threshold for investigation, which in turn shortens the length of time it takes to investigate.
What you need to tell us
Here is some of the key information we need (this list is not exhaustive).
- Fully completed referral form, including the part about referrals to third parties. Please be as specific as possible in the referral form in relation to what the concerns are about the worker. For example, if the concerns relate to inappropriate communication with someone using services, please specify the date(s) this occurred and exactly what the inappropriate communication was, including what was said, who it was said to, how many times it happened etc.
- Notes of relevant meetings, your investigation report and outcome of disciplinary hearings.
- Statements taken throughout the investigation.
- Contact details of any witnesses.
- Incident and accident reports.
- Any related policies and procedures.
- Job description.
- Training record.
- Any relevant medical report.
- Any previous concerns about the worker.
